Our Day program services offer a variety of supports for people with Intellectual or Development Disabilities. Services available may include a combination of supports such as community integration, employment opportunities, problem-solving and decision-making in everyday living, participating in community events and leisure activities, or assisting people to prepare for a vocational, recreational, or other community-based programs.

Community services are developed to provide supports to a child, adolescent or adult living with his or her family or in their own living environment. We serve individuals with a broad range of abilities and needs and our in-home services vary. The person and the family design the level of support and service that is needed.

Whether you're ill or you simply need some assistance around your home as you grow older, durable medical equipment can make sure your life isn't a burden. By providing a higher quality of life, you don't have to struggle anymore.
COMMUNITY-BASED RESIDENTIAL
COMMUNITY-BASED RESIDENTIAL
Community-based Residential services are provided in a home with other individuals. Supervision and support is provided 24 hours a day, 7 days a week as necessary. Availability is based on assessed need and may be limited due to waiver status.
Am I eligible for NC Innovations?
- Have an IDD diagnosis.
- Have limitations in three of six major activity areas (self-care, receptive and expressive language, capacity for independent living, learning, mobility, and self-direction and economic self-sufficiency).
- Need waiver services in order to keep living at home or to move out of an ICF/IDD group home/center.
- Be eligible for Medicaid.
- Live in a private living arrangement or a small facility (with no more than six residents).
Am I eligible for (b)(3) Services?
If you meet the requirements below, you may be eligible for (b)(3) services from provided by Elite Care Services:
- Have Medicaid insurance
- Live in one of the Cardinal Innovations counties
- Have intellectual disability/developmental disability, mental health or substance use service needs
- Meet other requirements specific to each service
Note: NC Innovations Waiver participants are eligible only for (b)(3) Psychiatric Consultation.
How do I get started?
If you already have NC Innovations and would like to use Elite Care Services as your provider, contact your Care Coordinator and they can coordinate Elite Care Services becoming your new provider.
If you do not have NC Innovations and would like to start the process, contact Cardinal Innovations registry department at 704-939-7980 and join the NC Innovations waitlist. This waiver has a waitlist due to limited Medicaid funds. Cardinal is required to keep the waiver waitlist for our counties, but only the state government has the power to approve new or additional spaces; these spaces are called “slots”. If a slot is not available, we can add you to the waiver’s waitlist, also called the “Registry of Unmet Needs.” We know this is frustrating. While you or your loved one wait for a slot on the waiver to open, you may be eligible for other resources. See (b)(3) services

If you are eligible for (b)(3) services and you already know which (b)(3) service you will like to receive, you can contact Elite Care Services directly at 704-982-4068. Elite Care Services will get in touch with Cardinal Innovations to start the eligibility process and ask for service approval.
